Instructions for filling up the Application Form
General Instructions

Fill up the application form in Block letters.
Incomplete applications will not be considered.
If you have furnished false information or to have suppressed material information, your name will be removed from our rolls at any time even after the selection.
Affix a recent passport size photograph in the space provided and enclose two additional copies and write your name on the back side of the photograph.
All correspondence should be addressed to the 'Principal' Skill Development Institute, Kochi.
Those who have already completed / attended one course from SDI Kochi is not eligible to apply for another course.

How to Fill the Application Form

Write full name in BLOCK LETTERS ONLY as it appears in your school certificate.
Aadhaar number and other information are mandatory.
Reservations applicable as per Government guidelines Aadhaar number and other information are mandatory.
Course preference:
a) Candidates with SSLC + ITI Welder / Fitter / Machinist / Turner can apply for Industrial Welding / Industrial Fitter.
b) Candidates with Plus2 + ITI Electrician / Wireman / Electronics Mechanic / Instrument Mechanic can apply for Industrial Electrician / Process Instrumentation.
Write all the qualifications that you have either appeared or passed as mentioned in column 10 of the application form.
